Key Notifiable Disease Trends

Enteric Diseases: There was an increase in the incidence of notifiable enteric diseases in June 2021 compared with June 2020. The lower number of cases seen in June 2020 coincides with New Zealand introducing stringent measures against COVID-19, such as social distancing, increased hygiene practices, working from home and children not attending school or pre-school. All these measures are also likely to prevent the spread of enteric diseases. 

COVID-19: There were 69 cases of COVID-19 reported in June 2021 (all confirmed), compared with 24 for the same month in 2020. Of the 69 cases, 68 were in managed isolation/quarantine (MIQ) facilities at the time of diagnosis and one was a historical community case. Three cases were hospitalised, and no deaths were reported. Legionellosis: There were 18 cases of legionellosis (16 confirmed and 2 under investigation) reported in June 2021, compared with 13 cases in June 2020. The serogroup was identified in 13 (81.3%) of the confirmed cases; eight were due to Legionella pneumophila (usually transmitted by inhalation of contaminated aerosols), four were due to L. longbeachae (commonly found in soil and potting mix) and one was due to L. micdadei. Yersiniosis: There were 150 cases of yersiniosis reported in June 2021 (all confirmed), compared with 77 for the same month in 2020. From January to June 2021 there have been 721 yersiniosis cases reported, compared with 466 for the same period in 2020. Almost half (65/150, 43.3%) of the June cases were from the Auckland region. One third (33.1%, 48/145) of cases with an ethnicity recorded were Asian. Yersinia enterocolitica biotype 2/3 serotype O:9 was identified in the majority (87/121, 71.9%) of cases that had been typed. 

OUTBREAKS 

COVID-19: An outbreak of COVID-19 involving three mariners at a Christchurch MIQ facility was reported in June. All three had travelled together from the Philippines to Christchurch and the first case was infectious on the flight. Whole genome sequencing and epidemiological evidence supported a single chain of transmission for the three cases. 

Respiratory illness: Public Health Units are asked to report any outbreaks of influenza-like illness/acute respiratory infections (including COVID-19) to EpiSurv as soon as possible. There were 21 respiratory illness outbreaks reported in June 2021 involving 515 cases. Fifteen outbreaks were reported as influenza-like illness (ILI) and six were acute respiratory infection (ARI). Rhinovirus and respiratory syncytial virus (RSV) were identified in four outbreaks each. No pathogen was identified in the remaining 13 ILI/ARI outbreaks. Eleven outbreaks were in childcare centres and 10 were in long-term care facilities. 

Salmonellosis: Since the last report, 10 further cases of Salmonella Enteritidis sequence type 11 have been identified as belonging to the Enteritidis_2019_C_01 cluster that was first reported in the March monthly report. From January to June 2021 there have been 47 cases reported, and since May 2019 the total number of cases associated with this strain is 114. The outbreak strain has been identified in poultry and egg farms. MPI announced an extension to its food testing programme to protect consumers on 22 July. This outbreak has been declared a Critical Food Safety Event, and MPI environmental investigations are ongoing.
